Son esprit baroudeur le pousse à s'engager dans les Marines mais sa demande est rejetée pour raison médicale. Il se rabat sur le cinéma en se spécialisant dans un genre musclé et machiste. Il signe un premier court métrage, I'm so bored qui attire l'attention de Roger Corman. Il devient un scénariste prolifique dans tous les genres : Viva Knievel de Gordon Douglas, Juge et hors-la-loi de Huston, Magnum Force de Clint Eastwood et Jeremiah Johnson de Sidney Pollack. Il signe son premier long métrage, Dillinger, retraçant la carrière du célèbre gangster en se signalant par sa violence. Le Lion et le Vent, inspiré d'une affaire diplomatique authentique au Maroc, confirme la puissance du metteur en scène. Il obtient en 1979 l'oscar du scénario pour Apocalypse Now de Coppola. Il poursuit sa carrière sur les deux fronts, avec une prédisposition pour le "romantisme guerrier", son œuvre fascisante démontre son sens de l'image choc et la rigueur de l'écriture. Scénariste, on lui doit Extrême Préjudice et Geronimo, tous deux de Walter Hill . Cinéaste, il s'illustre dans le genre musclé avec Conan le Barbare, L'Aube rouge et L'Adieu au Roi, notamment. Milius est également un producteur influent.
