Véritable pionnière, elle s'oriente très vite vers le cinéma, obtient la confiance de Léon Gaumont et réalise son premier film, La Fée aux Choux en 1896. Elle se verra ainsi confier l'essentiel de la production d'œuvres de fiction, abordant des sujets historiques (Le Courrier de Lyon), religieux (La Vie du Christ par deux fois), didactiques (Les Méfaits de la Tête de Veau, Les Dangers de l'Alcoolisme), sociaux (Le Voleur sacrilège) ou littéraires (La Esmeralda, Robert Macaire, Fanfan la Tulipe). Elle a cependant une nette préférence pour le fantastique. Elle forme le jeune Ferdinand Zucca qui deviendra son principal concurrent (avec Méliès), surtout après le départ de Zucca pour Pathé. Alice Guy supervise l'implantation de Gaumont en Angleterre. Elle épouse le caméraman Herbert Blaché et le couple tente sa chance aux États-Unis. Malgré des débuts prometteurs (Fra Diavolo, Le Puits et la Pendule), elle ne peut faire face à la concurrence et tombe dans l'oubli après 1920. Injuste au regard de son apport incontestable au septième art et aux nombreuses tentatives pour rendre le cinématographe plus attractif (elle met au point un procédé sonore dans ses Phonoscènes, expérience hélas sans lendemain). Son œuvre est redécouverte en 1953 lorsqu'elle reçoit la légion d'honneur. Caroline Huppert lui a consacré un téléfilm Elle voulait faire du Cinéma où Nathalie Baye incarne la première réalisatrice au monde.
Alice Guy est décédée à Mahwah (New Jersey), le 24 mars 1968.
